We partner with onside advocacy to provide lifestyle advice, social prescribing and wellbeing coaching as part of the Vale of Evesham.

What is social prescribing?

Social prescribing enables a GP to refer you to a link worker. Link workers give people time and focus on what matters to the person as identified through shared decision making or personalised care and support planning.

Social prescribing schemes can involve a variety of activities which are typically provided by voluntary and community sector organisations. Examples include volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.

Social prescribing particularly works for a wide range of people, including people:

with one or more long-term condition(s); who need support with their mental health; who are lonely or isolated; who have complex social needs which affect their wellbeing.

Recognising that people’s health is determined primarily by a range of social, economic and environmental factors, social prescribing seeks to address people’s needs in a holistic way. It also aims to support individuals to take greater control of their own health.

Lifestyle advice – make healthy lifestyle changes and need support and guidance on how to achieve them, such as:

  • Healthy eating 
  • Increasing physical activity 
  • Losing weight 
  • Stopping Smoking 
  • Reducing alcohol consumption 
  • Improving wellbeing

Wellbeing

This service is appropriate for patients presenting with mild mental health difficulties, such as: 

  • Stress and anxiety 
  • Self-esteem issues 
  • Grief 
  • Sleep problems 
  • Low mood 
  • Panic attacks 
  • Anger management 

This is a set programme consisting of up to 6 one to one sessions where patients have the opportunity to explore a range of evidence-based strategies and tools to be able to manage their own mental health issues.

This service is not suitable for patients with a history of chronic or more complex mental health issues or patients experiencing crisis.

Mental Health

The role works across the interface between Primary Care, Wellbeing Team, and Mental Health Services to provide advice and support, assessment, treatment, education, and problem-solving approaches.

The role also aims to improve the experience for our patients with mental health needs who would otherwise have seen a GP. The focus will be providing continuous assessment and treatment to people with mental health difficulties. This will include recovery co-ordination and care management functions, liaison with statutory and non-statutory organisations. 

Care-Coordinator can support/navigate patients and their families through the healthcare system, particularly patients living with frailty and long-term conditions. Connecting people to organisations for practical, social, and emotional needs. Signposting to activities,
groups and classes in the community.

If the doctor asks you to have an x-ray they will confirm if this will be in Gloucestershire of Worcestershire.

Out-Patient Appointments 

If you have not received an appointment following a referral please contact the booking office on 0300 4226872. You will have been given paperwork with all the details of the referral.

Travel Vaccinations

The following travel vaccines are available free on the NHS . Bredon Hill Surgery are signed up to provide the vaccination (immunisation) services.

The vaccines are:

polio (given as a combined diphtheria/tetanus/polio jab)
typhoid
hepatitis A
cholera
These vaccines are free because they protect against diseases thought to represent the greatest risk to public health if they were brought into the country.

If your travel itinerary requires you to have other vaccinations and advice and/or you are travelling within the next 6 weeks then you may be advised to go to a travel clinic.


If you are Travelling Abroad:

Please complete a travel form available from reception or print a copy (1 per person)

Return the completed form to the surgery – this needs to be at least 6 weeks before your travel date so that any vaccinations required can be given in time.

On returning the form to the surgery you will be asked to make a telephone appointment with a practice nurse. The nurse will ring you and discuss your vaccinations and if necessary advise if you need to use the services of a local travel clinic.

We can provide a print out of any previous vaccinations you may have had at your request – please contact reception.

Minor Surgery

 A clinic is held for minor surgical procedures, we hold a waiting list for this, please contact reception.

New Baby and mum check

Just had a baby? The doctor would like to see and your baby for a 6 week postnatal check.

Please call reception to book a face to face appointment for you and your baby. This will take approximately 30 minutes. This appointment should be at least a week before your baby has their first immunisations (see below)

Your baby will need immunisations at 8 weeks old – please call reception and book in to a baby clinic. We would like this to be no earlier than 8 weeks but within the 8th week after their birth

Midwife

Worcestershire Midwives

The Evesham Community Midwives Team is now known as the Abbey Team Midwives.

The Midwife team will be seeing the following women in clinic: 
Those that are newly pregnant for 1st appts (must be 9/10 weeks).
Those that are 28 weeks pregnant.
Those that are 36 weeks pregnant.

Clinical Pharmacist

The clinical pharmacist can help with a variety of tasks that include repeat authorisation of medication, medication reviews and dealing with queries regarding medication.

Smear Tests

You will recieve a letter to invite you to book an appointment.
